Information you provide. When you request an appointment, complete an intake or contact form, call us, or otherwise contact us, we may collect:

Information collected automatically. When you visit the Website, we may automatically collect your IP address, browser type, device and operating system, pages viewed, referring website and the date and time of your visit.

  • Schedule, confirm, and remind you of appointments

6. SMS/Text Messaging

Opt-in. If you provide your mobile number and check the optional SMS consent box on our intake or appointment form, you agree to receive text messages from Exact Eye Care. These may include appointment reminders and confirmations, order-ready notifications, intake follow-ups, and other service-related updates. Consent to receive text messages is optional and is not a condition of receiving care or making a purchase.

Frequency and cost. Message frequency varies. Message and data rates may apply.

Opt-out. You may opt out at any time by replying STOP to any message. You will receive one confirmation message, and no further messages will be sent unless you opt in again by replying START or re-enrolling.

No sharing of mobile information. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
